General Information


Consultations are offered to discuss important treatment information, health concerns, steps involved with procedures,financial/insurance information, and to answer any questions you may have. We recommend taking a few moments to write down any questions or concerns prior to your consultation to ensure your concerns have been addressed.


We do our best to schedule your visit(s) in an efficient and convenient manner. We are open Monday, Tuesday, and Thursday from 8:00am to 4:00pm and Fridays from 8:00am to 3:00pm. Consultations are typically in the afternoon so we may care for surgical patients in the morning hours.

We will coordinate and communicate with your referring doctor to provide optimal results in your overall oral health.

Dental Insurance:

We will file insurance claims for each visit to assist you. Please see the following insurance companies we participate with below:

We also accept most other insurance companies, as long as your policy allows you to select your own physician.

Medical Insurance:

Occasionally, medical insurance may pay a portion of your oral surgery needs. Most of these insurance policies require a referral from your primary care physician. A referral from a dentist is not adequate for medical insurance coverage. Obtaining a medical referral is your responsibility. We cannot obtain a referral for you, and the referral cannot be obtained retroactively. We are happy to see you if you do not have a referral, and payment will be due in full at the time of service. If you plan to submit your services to insurance, please provide all insurance plan identification, referral forms and all other necessary forms before being seen by the doctor. All co-payments and out-of-pocket fees are due at the time services are rendered.

At the time of your initial visit, your treatment plan will be established, and an estimate of fees will be provided to you. Payment is due at the time of service. Our office accepts cash, checks, Visa, Mastercard, Discover, and American Express.

Patients Using Insurance:

If you plan to use your insurance for part of your payment, we will file your claims for you as a courtesy. In the event an insurance carrier has not paid within 90 days of billing, any unpaid professional fees are due and payable in full from you.

Returned Check Fee:

A fee of $25.00 will be charged for any check returned for non-sufficient funds. In the event that two checks are returned for non-sufficient funds, all future payments will be required via cash at the time of service.


It is important for each patient to receive the personal time and attention they need for treatment. Help us continue to serve you best by keeping your scheduled appointments. Should you need to change your appointment date or time, we require a 48 hour notice to avoid a charge of $25.00. This ensures we are able to offer the most convenient times to you and our other patients.
